Oxycodone

Oxycodone is a prescription opioid used to treat moderate-to-severe pain, sold on its own (for example as OxyContin) and in combination products such as Percocet (with acetaminophen). It is an effective pain medicine but carries a high potential for misuse, dependence, and addiction, and it was central to the prescription-opioid wave of the overdose crisis. Taken as prescribed under medical supervision it has a role in pain management; misused — taken in higher doses, crushed to defeat extended-release formulations, or obtained outside the medical system — it is dangerous. As with other opioids, the illicit market is now dominated by counterfeit pills: tablets pressed to look like prescription oxycodone (such as "M30s") frequently contain fentanyl instead, in unpredictable and potentially lethal amounts.

Oxycodone is an opioid agonist: it binds to and activates mu-opioid receptors in the brain and spinal cord, blocking pain signals and producing pain relief along with sedation and, often, euphoria. Activating these receptors also triggers dopamine release that reinforces repeated use, and it suppresses the brain-stem centers that control breathing — which is why opioid overdose kills by stopping respiration. With repeated use the body develops tolerance and physical dependence, so more is needed for the same effect and stopping brings withdrawal. Dependence can develop even with legitimate prescribed use and is distinct from addiction, though it can contribute to it.

At therapeutic doses oxycodone relieves pain and commonly causes drowsiness, constipation, nausea, dizziness, dry mouth, and a sense of calm or euphoria. As the dose rises — or when potency is unknown, as with counterfeit pills — the dangerous effects emerge: pinpoint pupils, heavy sedation, confusion, and breathing that becomes slow and shallow or stops. With repeated use, tolerance and physical dependence develop; stopping after regular use brings opioid withdrawal — muscle and bone aches, restlessness, insomnia, sweating, nausea, diarrhea, and strong cravings — intensely uncomfortable but not usually directly life-threatening on its own.

Oxycodone's central danger is respiratory depression: too much slows or stops breathing, and the margin shrinks sharply with higher doses, with reduced tolerance (for example after a period of abstinence), and when combined with other depressants. Mixing oxycodone with alcohol, benzodiazepines, or other opioids compounds the suppression of breathing and is a frequent factor in fatal overdoses. The counterfeit-pill problem makes illicit oxycodone especially deadly: pills sold as oxycodone are often fentanyl in disguise, far more potent and impossible to gauge by appearance. Naloxone (Narcan) reverses an oxycodone or fentanyl overdose, is available over the counter in the U.S. since 2023, may require more than one dose when fentanyl is involved, and is safe to give even if opioids turn out not to be the cause. Overdose signs are unresponsiveness, slow or stopped breathing, gurgling or snoring sounds, and blue or grey lips or fingertips — give naloxone if available and call 911 immediately.
