Morphine

Morphine is a naturally occurring opioid extracted from the opium poppy, and it remains one of the most important medicines for treating moderate-to-severe pain — for example after surgery, during cancer care, or in palliative settings. It is the reference drug against which other opioids' potency is measured. Because it is a strong opioid, it carries the same core dangers as the rest of the class: it can suppress breathing, it produces tolerance and physical dependence with repeated use, and it can be fatal in overdose. Most morphine use is medical and supervised, but it is also misused, and like other prescription opioids it appears on the illicit market — where, as with all street opioids, there is a risk that what is sold as one thing is actually mixed with or replaced by fentanyl.

Morphine is an opioid agonist: it binds to and activates mu-opioid receptors in the brain and spinal cord, blocking the transmission of pain signals and producing pain relief along with sedation and, often, a sense of euphoria. Activating these receptors also triggers dopamine release that reinforces repeated use, and — critically — it suppresses the brain-stem centers that control breathing. That respiratory depression is how opioid overdose kills. With repeated use the body adapts, so more is needed for the same effect (tolerance) and the body comes to depend on the drug's presence (physical dependence); stopping then brings withdrawal. Tolerance to the breathing-suppressing effect lags behind tolerance to the desired effects, which is part of why dose escalation is dangerous.

At therapeutic doses morphine relieves pain and commonly causes drowsiness, a sense of calm or euphoria, constipation, nausea, dry mouth, sweating, and small (constricted) pupils. As the dose rises, the dangerous effects emerge: heavy sedation, confusion, and breathing that becomes slow and shallow or stops. With repeated use, tolerance and physical dependence develop, and stopping after regular use brings an opioid withdrawal syndrome — muscle and bone aches, restlessness, insomnia, sweating, nausea, vomiting, diarrhea, and strong cravings — deeply unpleasant but not usually directly life-threatening on its own. Dependence can develop even with appropriate prescribed use and is distinct from addiction, though it can contribute to it.

Morphine's central danger is respiratory depression: too much slows or stops breathing, and the margin narrows sharply with higher doses, with reduced tolerance (for example after a period of not using), and when morphine is combined with other depressants. Mixing morphine with alcohol, benzodiazepines, or other opioids compounds the suppression of breathing and is a frequent factor in fatal overdoses. Overdose presents as unresponsiveness, slow or stopped breathing, gurgling or snoring sounds, pinpoint pupils, and blue or grey lips or fingertips. Naloxone (Narcan) reverses an opioid overdose, is available over the counter in the U.S. since 2023, and is safe to give even if opioids turn out not to be the cause — give it if available and call 911 immediately.
