Zolpidem

Zolpidem, sold under the brand name Ambien, is a prescription sedative-hypnotic used for short-term treatment of insomnia. It is often called a 'Z-drug' — it is not a benzodiazepine chemically, but it acts on the same brain system and shares many of the same risks. It is intended for short-term use because tolerance and dependence can develop, and it is known for an unusual and serious effect: complex sleep behaviors such as sleep-walking, sleep-eating, and even sleep-driving, performed with no memory of them afterward. Like other CNS depressants, it is dangerous in combination with alcohol or other sedatives. It is widely prescribed and also misused.

Zolpidem acts on the same GABA-A receptor system as benzodiazepines, enhancing the calming effect of the neurotransmitter GABA to slow brain activity and promote sleep, though it binds somewhat more selectively to the receptor subtypes associated with sedation. Because it works through this same inhibitory system, it carries the class risk of dangerous CNS and breathing suppression when combined with other depressants. The drug is relatively short-acting, designed to help with sleep onset, but its effects on the brain during partial wakefulness are thought to underlie the complex sleep behaviors it can cause. Repeated use can lead to tolerance and dependence.

Zolpidem produces drowsiness and sleep, and common effects include grogginess, dizziness, and impaired coordination, sometimes lingering into the next morning and impairing driving. The drug's hallmark adverse effect is complex sleep behavior — walking, eating, making phone calls, having conversations, or driving while not fully awake, with no memory of it — which can be dangerous and has prompted strong regulatory warnings. Some people experience memory gaps, confusion, or unusual behavior. With repeated use, tolerance and dependence can develop, and stopping after regular use can cause rebound insomnia and withdrawal. Effects are intensified by alcohol and other sedatives.

Two specific dangers stand out. First, complex sleep behaviors: zolpidem can cause people to carry out activities — including driving — while not fully awake and with no memory afterward, and serious injuries and deaths have occurred, which is why regulators warn against use in anyone who has experienced such an episode. Next-morning impairment can also make driving dangerous even when a person feels awake. Second, combination with other depressants: taken with alcohol, opioids, benzodiazepines, or other sedatives, zolpidem compounds CNS and breathing suppression and can be dangerous or fatal. Tolerance and dependence can develop with continued use, and abrupt discontinuation can cause withdrawal, so it should be stopped under medical guidance. Any pill obtained outside a licensed pharmacy can be counterfeit and may contain other substances, including fentanyl, so naloxone (over the counter in the U.S. since 2023) is worth having on hand.
