Diazepam

Diazepam, widely known by the brand name Valium, is a long-acting benzodiazepine used for anxiety, seizures, muscle spasm, and alcohol withdrawal. It is an effective, long-established medication, but like all benzodiazepines it carries a significant risk of tolerance and dependence with regular use, and it is dangerous when combined with other central-nervous-system depressants. Its long duration of action means it stays in the body for an extended time, which affects both how it is used medically and how dependence and withdrawal play out. It is also misused, and counterfeit benzodiazepine pills sold illicitly are a growing hazard.

Diazepam enhances the effect of GABA, the brain's main inhibitory neurotransmitter, by making GABA's calming signal more effective. This slows central nervous system activity, which reduces anxiety, relaxes muscles, suppresses seizures, and produces sedation. The same mechanism is why benzodiazepines are dangerous in overdose and especially in combination with other depressants: when CNS activity is suppressed too far, breathing slows. With repeated use the brain adapts to the drug's presence, producing tolerance and physical dependence, so the nervous system becomes over-excitable if the drug is suddenly removed — the basis of a withdrawal syndrome that can be medically dangerous. Diazepam's long half-life means it and its active breakdown products linger in the body.

Diazepam reduces anxiety and produces sedation, muscle relaxation, and a sense of calm. Common effects include drowsiness, fatigue, poor coordination, slurred speech, and impaired memory and concentration; some people experience paradoxical agitation. It impairs driving and tasks needing alertness, and its long duration means effects and grogginess can persist into the next day. With repeated use, tolerance develops and dependence can form; stopping abruptly after regular use can cause rebound anxiety and insomnia and, in more serious cases, a dangerous withdrawal that includes seizures. Because it is long-acting, withdrawal tends to start later and last longer than with short-acting benzodiazepines.

Two dangers stand out. The first is combination with other depressants: taken with opioids, alcohol, or other sedatives, diazepam compounds the suppression of breathing and is a frequent factor in fatal overdoses — regulators carry explicit warnings about benzodiazepine-and-opioid combinations. The second is withdrawal: after regular use, stopping suddenly can trigger seizures and other dangerous symptoms, so discontinuation should be done gradually under medical supervision, never abruptly and alone. A third growing risk is the counterfeit market — pills sold illicitly as Valium or other benzodiazepines may contain different benzodiazepines, other drugs, or fentanyl in unpredictable amounts, so naloxone (over the counter in the U.S. since 2023) is worth having on hand because these pills can cause an opioid overdose.
