Lorazepam

Lorazepam, sold under the brand name Ativan, is a benzodiazepine used for anxiety, for certain seizure emergencies, and as a sedative before medical procedures. It is intermediate-acting and, like all benzodiazepines, effective but carrying a real risk of tolerance and dependence with regular use. The standard benzodiazepine cautions apply: it is dangerous combined with opioids, alcohol, or other depressants, and stopping abruptly after regular use can be medically dangerous. It is widely prescribed and also misused, and counterfeit benzodiazepine pills are an increasing illicit-market hazard.

Lorazepam enhances the calming effect of GABA, the brain's main inhibitory neurotransmitter, slowing central nervous system activity to reduce anxiety, suppress certain seizures, and produce sedation. As with the whole class, this CNS suppression is why benzodiazepines are dangerous in overdose and especially in combination with other depressants, where breathing can slow too far. Repeated use leads the brain to adapt, producing tolerance and physical dependence; abrupt removal then leaves the nervous system over-excited, the basis of a potentially dangerous withdrawal syndrome. Lorazepam's intermediate duration shapes how quickly its effects and withdrawal appear.

Lorazepam reduces anxiety and produces sedation, drowsiness, and muscle relaxation. Common effects include fatigue, dizziness, poor coordination, slurred speech, and impaired memory and concentration; some people experience paradoxical agitation. It impairs driving and tasks requiring alertness. With repeated use, tolerance develops and dependence can form; stopping abruptly after regular use can cause rebound anxiety and insomnia and, in more serious cases, a dangerous withdrawal that includes seizures. Its intermediate half-life means withdrawal can begin relatively soon after the last dose compared with longer-acting benzodiazepines.

Lorazepam carries the two central benzodiazepine dangers. First, combination with other depressants: taken with opioids, alcohol, or other sedatives, it compounds the suppression of breathing and is a frequent factor in fatal overdoses, with explicit regulatory warnings about benzodiazepine-and-opioid combinations. Second, withdrawal: after regular use, stopping suddenly can trigger seizures and other dangerous symptoms, so it must be tapered gradually under medical supervision rather than stopped abruptly and alone. The counterfeit market adds a third danger — pills sold illicitly as Ativan may contain other benzodiazepines, unrelated drugs, or fentanyl in unpredictable amounts, so naloxone (over the counter in the U.S. since 2023) is worth having because these pills can cause an opioid overdose.
