Kratom (Mitragyna speciosa) is controlled in Italy — the plant was inserted into Table I of the narcotics tables (DPR 309/90) by Ministry of Health decree of 1 August 2016; mitragynine (the cited basis) and 7-hydroxymitragynine are controlled as constituents of the scheduled plant.
Consequences if caught
Table I narcotic offences under DPR 309/90: personal-use possession is administrative (art. 75), while dealing and trafficking are criminal (art. 73).

No lawful medical route — kratom is a Table I narcotic plant with no authorised medicinal product.
