Methadone
Methadone is a long-acting opioid used to treat opioid addiction and pain; it is effective and life-saving in treatment but dangerous in overdose, especially early in use.
Overview
Methadone is a long-acting synthetic opioid with two main medical uses: treating opioid use disorder (as a maintenance medication that prevents withdrawal and reduces craving) and managing severe pain. As a treatment for opioid addiction it is well-established and life-saving — it lets people stabilize without the cycle of withdrawal and is delivered through regulated programs. At the same time it is a full opioid, so it carries the class risks of respiratory depression and dependence, and it has a particular hazard: because it is long-acting and accumulates in the body, its dangers are easy to underestimate, and overdose risk is highest early in treatment before the right dose is established. It is both one of the most useful tools against the opioid crisis and a drug that demands careful medical supervision.

Chemistry & mechanism of action
Methadone is a mu-opioid receptor agonist like other opioids, producing pain relief and preventing opioid withdrawal, but it differs in two important ways: it acts slowly and lasts a long time in the body. This long duration is exactly what makes it useful for treating addiction — it provides steady opioid-receptor activity that suppresses withdrawal and craving without the rapid peaks and troughs of shorter-acting opioids. But the same property makes it dangerous if dosed carelessly: methadone can build up over repeated doses, and its effects (including breathing suppression) can outlast what a person feels, so the risk of overdose is greatest while a dose is being established. It also can affect heart rhythm (QT prolongation) in some people. Tolerance and physical dependence develop with use.

Effects
Used as prescribed, methadone prevents opioid withdrawal and craving without producing a strong high, which is what allows people in treatment to function normally. Common effects include drowsiness, constipation, sweating, dry mouth, and nausea, especially when starting. Because it is long-acting, sedation and breathing suppression can come on gradually and persist. At higher-than-tolerated doses the opioid dangers dominate: heavy sedation, confusion, and slow, shallow breathing. Stopping after regular use produces opioid withdrawal, which with methadone tends to be slower in onset and more prolonged than with shorter-acting opioids. In treatment settings, the goal is a steady dose that produces neither withdrawal nor intoxication.

Risks & harms
Methadone's central danger is respiratory depression, and its long-acting, accumulating nature makes the timing of risk distinctive: overdose risk is highest in the first days to weeks of use, before a stable dose is reached, because the drug builds up and its breathing-suppressing effect can outlast the felt effect. The risk rises sharply when methadone is combined with other depressants — alcohol, benzodiazepines, or other opioids — which is a frequent factor in methadone deaths. Methadone can also affect heart rhythm in some people. None of this is a reason to avoid methadone treatment, which is proven to save lives; it is a reason it must be taken exactly as prescribed and not combined with other depressants. Naloxone (Narcan) reverses a methadone overdose but, because methadone is long-acting, repeat doses and monitoring are often needed; it is over the counter in the U.S. since 2023 and safe to give regardless — give it and call 911 for slow or stopped breathing or unresponsiveness. Legal status (US)
In the United States, methadone is a Schedule II controlled substance under the Controlled Substances Act. When used to treat opioid use disorder it is dispensed through federally regulated opioid treatment programs under specific rules; when prescribed for pain it is handled like other Schedule II opioids. Using methadone outside these channels, or possessing it without a valid prescription, is illegal. Internationally, methadone is controlled under the United Nations Single Convention on Narcotic Drugs and is recognized by the World Health Organization as an essential medicine for opioid dependence, so most countries permit its medical and treatment use while controlling it. If it’s too intense
If an experience becomes overwhelming, the goal is to stay safe and let it pass — most difficult experiences ease as the drug wears off.
- Get to a calm, safe space with someone you trust who is sober and can stay with you.
- Cool down if you’re overheating — move somewhere cool, remove extra layers, rest. Overheating is especially a risk with stimulants and MDMA.
- Sip water to thirst — but don’t over-hydrate. Drinking large amounts of plain water (especially after MDMA) can dangerously dilute your blood sodium (hyponatremia). Electrolytes help more than volume.
- Slow your breathing — long, slow exhales help settle a racing heart and anxiety.
- A sugary drink, fruit juice, or a snack can ease shakiness and the anxiety that comes with low blood sugar.
- Do not take more, and do not add another substance to manage it. Redosing or adding something else (including a sedative like a benzodiazepine) can make things worse, not better.
With opioids, slowed or stopped breathing is the emergency — if available, give naloxone and call 911 immediately; it can be given while you wait for help.
