Ephedrine
Ephedrine is a stimulant and decongestant, related to amphetamine, found in the ephedra plant and in some cold medicines. It is not a controlled substance, but pure ephedrine is a DEA List I chemical (a methamphetamine precursor), OTC sales are restricted under the Combat Methamphetamine Epidemic Act, and ephedra weight-loss supplements are FDA-banned over cardiac risks.
Overview
Ephedrine is a sympathomimetic stimulant and bronchodilator derived from Ephedra plants (ma huang) and also made synthetically. Medically it has been used as a decongestant, to treat low blood pressure during anesthesia, and for asthma. It became widely known in weight-loss and 'energy' supplements before those were banned in the US, and it is chemically related to amphetamine and to pseudoephedrine.

Chemistry & mechanism of action
Ephedrine stimulates the sympathetic nervous system, both by triggering release of norepinephrine and by directly activating adrenergic receptors. This raises heart rate and blood pressure, opens the airways (bronchodilation), and constricts blood vessels. Its stimulant and appetite-suppressing effects, and its cardiovascular strain, all flow from this broad adrenergic activity.

Effects
Effects include increased alertness and energy, appetite suppression, decongestion, and opened airways, alongside raised heart rate, higher blood pressure, jitteriness, anxiety, insomnia, and palpitations. At higher doses the cardiovascular and neurological stimulation becomes pronounced. Tolerance to the stimulant effects can develop with repeated use.

Risks & harms
Ephedrine's central danger is cardiovascular: it raises heart rate and blood pressure and has been linked to heart attack, stroke, dangerous arrhythmias, and death, particularly at high doses, with heavy use, in people with underlying heart conditions, or when combined with other stimulants such as caffeine — the combination common in the banned weight-loss supplements. The FDA banned ephedra (ephedrine-alkaloid) dietary supplements in the US because the risks outweighed benefits. Combining ephedrine with certain antidepressants (MAO inhibitors) can cause a dangerous spike in blood pressure. Overheating and agitation can occur with heavy use. Because pure ephedrine is a methamphetamine precursor, it is legally restricted (see the legal section), and products bought outside regulated channels may be mislabeled or contaminated. Legal status (US)
In the United States, ephedrine is not a controlled substance and is not placed in a DEA schedule, but it is regulated on two fronts. Pure ephedrine is a DEA List I chemical because it is a precursor used to manufacture methamphetamine, and over-the-counter products containing it are 'scheduled listed chemical products' under the Combat Methamphetamine Epidemic Act of 2005: sold from behind the counter, requiring photo ID and a signed logbook, with purchase caps (3.6 grams per day; 9 grams per 30 days in-store). Exceeding limits or possessing it to manufacture drugs carries serious federal penalties. Separately, the FDA has banned ephedra-alkaloid dietary supplements. If it’s too intense
If an experience becomes overwhelming, the goal is to stay safe and let it pass — most difficult experiences ease as the drug wears off.
- Get to a calm, safe space with someone you trust who is sober and can stay with you.
- Cool down if you’re overheating — move somewhere cool, remove extra layers, rest. Overheating is especially a risk with stimulants and MDMA.
- Sip water to thirst — but don’t over-hydrate. Drinking large amounts of plain water (especially after MDMA) can dangerously dilute your blood sodium (hyponatremia). Electrolytes help more than volume.
- Slow your breathing — long, slow exhales help settle a racing heart and anxiety.
- A sugary drink, fruit juice, or a snack can ease shakiness and the anxiety that comes with low blood sugar.
- Do not take more, and do not add another substance to manage it. Redosing or adding something else (including a sedative like a benzodiazepine) can make things worse, not better.
With stimulants, overheating and a racing heart are the main concerns — get somewhere cool, stop any physical activity, and don't take more.
