5-MeO-DMT is an extremely potent, fast, short-acting psychedelic tryptamine found in certain plants and in the venom of the Colorado River toad. It differs from DMT and psilocybin in acting strongly at the serotonin 5-HT1A receptor. Its risks include overwhelming experiences, dangerous MAOI interactions, and, in toad-derived material, cardiotoxins.

Overview

5-MeO-DMT (5-methoxy-N,N-dimethyltryptamine) is a naturally occurring psychedelic of the tryptamine class, first synthesized in 1936. It is found in a range of plants and, famously, in the venom secreted by the Colorado River toad (Incilius alvarius, formerly Bufo alvarius), which is why it is sometimes called "toad" or "the power." It is closely related to DMT and psilocybin but is distinct in its effects and is often described as one of the most powerful and overwhelming psychedelics. It is typically vaporized and inhaled, producing a very rapid and intense experience, and has drawn research interest for possible rapid antidepressant effects.

Chemistry & mechanism of action

5-MeO-DMT is a serotonin receptor agonist that acts at both the 5-HT2A receptor (the classic psychedelic target) and, distinctively, very strongly at the 5-HT1A receptor — with functional dominance at 5-HT1A that sets it apart from DMT and psilocybin, whose effects are led by 5-HT2A. This different receptor balance is thought to underlie its unusual, often less visual and more all-encompassing phenomenology. Like DMT, it is metabolized by the enzyme monoamine oxidase (MAO), which is why combining it with an MAO inhibitor dramatically potentiates and prolongs its effects and raises the risk of serotonin toxicity.

Effects

When vaporized and inhaled, 5-MeO-DMT comes on within seconds to a couple of minutes and is short-lived, typically lasting well under an hour. Users describe an extraordinarily intense experience — a rapid, often complete dissolution of the sense of self rather than the rich visual imagery associated with DMT. Reported effects also include amplified emotion, distortions of perception and time, and, physically, nausea, vomiting, chest pressure, and elevated heart rate and blood pressure.

Risks & harms

The sheer intensity of 5-MeO-DMT is itself a risk: overwhelming fear, panic, confusion, loss of motor control, and, rarely, a persisting destabilization can occur, and the experience can be psychologically hazardous for people with a history of psychosis. The MAOI interaction is a major danger — combining 5-MeO-DMT with harmala alkaloids (as in ayahuasca preparations) or with serotonergic antidepressants can cause life-threatening serotonin syndrome. When the drug is obtained from toad venom rather than synthesized, there is an additional hazard: the raw venom also contains cardiotoxic compounds (bufadienolides, chemically similar to the heart drug digoxin) that can cause dangerous heart-rhythm disturbances, a risk that is especially acute for anyone with heart disease or on cardiovascular medication. Its cardiovascular stimulation is a concern for people with hypertension or heart conditions.

If it’s too intense

If an experience becomes overwhelming, the goal is to stay safe and let it pass — most difficult experiences ease as the drug wears off.

  • Get to a calm, safe space with someone you trust who is sober and can stay with you.
  • Cool down if you’re overheating — move somewhere cool, remove extra layers, rest. Overheating is especially a risk with stimulants and MDMA.
  • Sip water to thirst — but don’t over-hydrate. Drinking large amounts of plain water (especially after MDMA) can dangerously dilute your blood sodium (hyponatremia). Electrolytes help more than volume.
  • Slow your breathing — long, slow exhales help settle a racing heart and anxiety.
  • A sugary drink, fruit juice, or a snack can ease shakiness and the anxiety that comes with low blood sugar.
  • Do not take more, and do not add another substance to manage it. Redosing or adding something else (including a sedative like a benzodiazepine) can make things worse, not better.

With psychedelics, fear and confusion are usually temporary. Change your surroundings — calmer light, quiet music, a trusted person — and remind yourself it will lift as the drug wears off.

Call 911 (or Poison Control, 1-800-222-1222) right away for chest pain, a very high body temperature, a seizure, unconsciousness, or severe confusion. These are medical emergencies, not something to wait out.
